Purpose
The Transitions Strategy sets out a partnership approach to ensure a rich and fulfilling transition for young people from childhood to adulthood. It includes the way we work with our young people and their parents/carers across health and social care.
Cabinet is asked to note the engagement which has taken place with stakeholders to co-produce the refreshed Strategy.
