To demonstrate how, through integrating targeted health and social care services for older people, opportunities to improve outcomes for citizens and the delivery of efficiencies can be maximised. To specifically set out the financial benefits for BCC and how this impacts on the Medium Term Financial Plan. To seek permission for BCC to lead a procurement exercise on behalf of Partners to maximise the opportunity.
